2. Complete Your Intake Forms
Before your first appointment you will receive a secure link to your SimplePractice patient portal where you will complete your intake paperwork digitally. This includes your health history, consent forms, and payment, all handled electronically before your session so your appointment time is spent entirely on your care.

3. Meet Your Doctor of Physical Therapy
At your scheduled appointment time open your secure video link and connect directly with your Doctor of Physical Therapy. Your provider will conduct a thorough assessment of your spine including a detailed health history review, movement analysis, and functional evaluation.

4. Get Your Personalized Plan
Based on your comprehensive evaluation your provider will explain your condition in clear terms and deliver a personalized rehabilitation plan built specifically around your condition, your body, and your lifestyle. Your home exercise program will be delivered digitally through your patient portal immediately following your session.

5. Follow Your Plan

What You Need to Get Started
Any smartphone, tablet, or computer with a functioning camera and microphone. No app download required — your session opens directly in your browser.
1. A Device with A Camera
A standard home broadband connection is all you need. We recommend at least 10 Mbps download and upload speed for the best video quality.
2. A Reliable Internet Connection
Wear comfortable clothing that allows freedom of movement. Have a clear space available so your provider can observe your movement patterns during assessment.
